Client’s Rights and Responsibilities

Although there is a standard of care applied to every client, you will be evaluated and cared for appropriately as an individual by the staff at Maternidad La Luz. As a client of Maternidad La Luz, it is your right and responsibility to understand all of the care that is offered to you. You have the right to question and/or decline any service or test that is offered. However, declining certain services or tests may make you ineligible to continue receiving care at Maternidad La Luz.

You have the responsibility to take care of yourself to the best of your ability through good nutrition, exercise, rest, and coming to your scheduled appointments and childbirth education.

Important Client Information
In order to be accepted as a client at the center you must meet the requirements set out by the Texas Department of State Health Services. This document defines our scope of practice. These rules help protect you and your baby. The law may require that you have a consultation with a Texas licensed physician or be referred to a hospital for certain conditions. Maternidad La Luz cannot guarantee that your baby will be born at the center or in the United States. In an emergency, you will be transported to the nearest hospital, usually University Medical Center (UMC). We make every attempt to accompany our clients to the hospital in the event of a transport. If transported, financial arrangements with the hospital are the responsibility of the client. The hospital staff will evaluate you and decide if need to be admitted or if you may be permitted to return to the care of Maternidad La Luz or your physician.

Maternidad La Luz does not allow videos of births. We will help you with still photos of your baby’s birth if you bring your own camera.

Maternidad La Luz may need to consult by phone or in writing with a health care provider regarding you or your baby. This may include your personal physician or a health care provider that we work with.

Neither Maternidad La Luz nor its staff carries liability or malpractice insurance.



 Los Derechos y Responsabilidades de Cliente

individualmente por las parteras de Maternidad La Luz y recibirá los cuidados adecuados
para usted. Mientras sea cliente de Maternidad La Luz, es su derecho y responsabilidad
entender el cuidado ofrecido. Tiene el derecho de cuestionar y/o anunciar cualquier
servicio o prueba ofrecida. Negando ciertos servicios o pruebas puede hacerle inelegible
para continuar a nuestro cuidado.

Usted tiene la responsabilidad de cuidarse lo mejor posible a través de una dieta
equilibrada, ejercicio, descanso, y venir a sus citas y clases prenatales.

Información Importante al Cliente
Para poder aceptarla como cliente en la clínica, debe cumplir con ciertos requisitos que
piden el Departamento de Salud de Texas. Este documento define nuestra esfera de
competencia. Estos requisitos fueron hechos para proteger su salud y la de su bebé. La ley
de partería puede requerir que usted sea enviada con un médico o a un hospital para tratar
ciertas condiciones.

Maternidad La Luz no puede garantizarle que su bebé nazca en la clínica o en los Estados
Unidos. En caso de una emergencia se le llevara al hospital más cercano, usualmente el
Centro Universitario Medico (UMC). Tratamos de acompañar nuestras clientes en caso de
transporte al hospital. El plan de pago con el hospital será la responsabilidad de la cliente.
El personal del hospital decidirá si usted necesita estar internada o si podrá regresar al
cuidado de Maternidad La Luz o su médico.

Entiendo sobre la póliza de la maternidad que no podré tomar fotos durante el parto.
